Trust
The Benefits
Trust is a contractual benefit that collects money from the district
and returns it to JTA members in the form of reimbursements.
Medical, dental, eyeglass, and prescription drug payments may
be reimbursed when the proper proof is sent to the director of
the fund.
The
NYSUT Legal Plan
The
NYSUT Legal Plan offers access to legal assistance for personal legal matters.
The Legal Service Plan is provided by Feldman,
Kramer & Monaco,
P.C. and is administered by NYSUT Member Benefits Trust.
Vision
Program
Our Vision
Program (GVS Bronze Plan #8610/8611) entitles you and your
family to the following vision benefits:
- Comprehensive
eye exam
- Selection
of various frames up to a retail value of $125.00
- Selection
of lenses: single vision, conventional bifocal, conventional
trifocal
- Contacts:
standard soft daily wear or extended wear spherical or a 6
month supply of basic disposable contact lenses
- Loss protection
program: allows for the replacement of benefits package for
a co-pay of $25.00

Medical
Excess Medical,
the part of our medical insurance the district doesn’t
include is covered by the Benefits Trust and is managed by Fitzharris & Company. From
this Web site you can download claim forms and access the providers
within the network.
According to
our current contract*, the amount of medical insurance covered
by the district will change over the next 5 years.

*PLEASE read
section D of the MOA very carefully as our increased contribution
and our out of pocket cost was handled quite creatively! It’s
not as bad as it looks.
Excess
Dental
The district
provides 90% of the premium payment for our dental coverage.
Excess dental is covered by the Benefits Trust and is managed
by Guardian.

Educational
Development Fund
JTA members
can apply for up to $200.00 of reimbursement (beyond what the
district reimburses) for expenses incurred attending approved
educational conferences and activities. For example, generally
speaking the district will reimburse you $50.00 of the registration
fee for a conference. If however, the conference registration
is $150.00 you can apply to the JTA for the difference.
How to apply
Send all of
the following to the JTA office:
- a copy of
the district approved Travel or Conference Order
- a completed
Educational Development Fund Conference Reimbursement Application
- a copy of
any receipts you want reimbursement for
- and a brief
report of what the conference covered
NOTE: You may
submit as many applications for reimbursement as you wish, but
the total reimbursement is $200.00 per school year. Each application
must be submitted separately and contain all the documents listed
below. Applications are reviewed three times each year.
